Terms of Service & Refund Policy

Scope of Services

Insight Disability Solutions (“we,” “our,” “us”) provides professional virtual support to clients across Ontario and Nova Scotia for the following services:

  • Accessible Parking Permit (APP) applications

  • Student Accessibility Forms (academic accommodations)

  • Caregiver & Dependency Forms

  • Housing & Community Support Forms

  • Disability Tax Credit (DTC) assessments and T2201 form completion

  • Adaptive Equipment & Vehicle Accessibility Support

Important Professional Scope:

  • We do not diagnose or treat any medical or psychological condition.

  • Services are based solely on documentation provided by the client or a qualified professional.

  • We verify, review, and complete forms within our professional scope, providing guidance and signatures where authorized.

  • All accommodations or recommendations must be supported by the documentation submitted.

  • Clients are responsible for providing official forms and verified documentation required by schools, institutions, or governing bodies.

Client Responsibilities

By using our services, you agree to:

  • Provide accurate and truthful information.

  • Supply all required documentation to support your request (medical, psychological, educational, or professional evaluations).

  • Complete all required consent forms prior to any services being provided.

  • Attend virtual appointments as scheduled to review documentation and confirm details.

Payment Terms

  • Services are fee-for-service. Payment must be received before we begin work.

  • Payments can be made via WIX form integration, e-transfer, or other agreed-upon methods.

  • Fees are non-refundable once services have commenced or a form has been completed within professional scope.

Refund Policy

  • Refunds will only be issued if:

    • The service has not yet begun (no review, verification, or form completion).  Yet once a form/request is submitted, there is no refund.

  • Once a virtual appointment has occurred, or the form has been reviewed or signed, no refunds will be issued.

  • Any disputes will be addressed according to the payment method’s policies, but the client acknowledges that services rendered are considered complete once work has been performed.

Consent and Authorization

By using our services, you provide explicit consent for:

  • Insight Disability Solutions to review your submitted documentation.

  • Completing official forms within professional scope based on the documentation provided.

  • Contacting you via email, phone, or virtual platforms to schedule appointments, clarify details, and confirm consent.

 

Important: All services are virtual unless otherwise agreed. Requests submitted outside business hours will be reviewed on the next business day.

Privacy & Data Protection

  • All client information is handled in accordance with PHIPPA (Personal Health Information Protection Act) and Canadian privacy laws.

  • We may collect, store, and use your personal information only for the purpose of providing services.

  • File uploads, forms, and documentation are stored securely using:

    • WIX secure database

    • Jane App EMR for internal tracking

    • eFax for document receipt

  • We do not share your information without your consent except as required by law or necessary for completing official forms.

Limitation of Liability

  • Insight Disability Solutions is not responsible for delays, rejection, or errors caused by:

    • Incomplete or inaccurate documentation provided by the client.

    • Decisions made by schools, institutions, or government agencies.

  • Clients acknowledge that Insight Disability Solutions provides guidance and professional completion of forms, but final approval is at the discretion of the institution or governing body.

Acceptance of Terms

By submitting a form, scheduling a virtual appointment, or making a payment, the client acknowledges:

  • They have read, understood, and agreed to these Terms of Service and Refund Policy.

  • They accept all conditions including fee-for-service, professional scope limitations, and privacy practices.

  • They cannot hold Insight Disability Solutions liable for any outcomes outside our professional scope.
