How the Process Works
Get Started & Payment
To begin, clients contact us by phone or email to request services.
Secure payment is required before any submission is reviewed.
Please be prepared to provide:

Once payment is received, we guide you through the next steps and provide any required forms or secure upload instructions.
Submission of Forms & Documentation
Clients submit official forms and all supporting documentation through a secure portal or eFax.
Documentation is reviewed to ensure it supports the requested accommodations or certification.
Virtual Clinical Review
A virtual appointment with a Nurse Practitioner is scheduled to:

Review submitted documentation

Confirm consent and eligibility

Clarify functional limitations or accommodation needs
(No in-person visit is required.)
Form Completion
Using verified documentation and institutional-aligned functional language, required forms are completed and signed within professional scope and regulatory standards.
Delivery & Ongoing Support
Completed documentation is returned to the client or uploaded directly to the required portal, where applicable.
Follow-up support is provided if clarification or revisions are requested by the receiving institution.
Pricing – Student Accessibility Support
At Insight Disability Solutions, we provide licensed healthcare provider-led virtual assessments and professional documentation to support student accessibility and accommodation requests.
Standard Rate
$125 per form
Includes:

Multiple Forms / Renewals

Two forms submitted together (same assessment & time period): $225
-
Three or more forms submitted together (same assessment): $50 per additional form
-
Renewal or updated form (no significant changes; within 6 months of original assessment date; does require brief re-assessment): $75 per 1st document; $30 per additional
Packages and pro-rated fees apply only if forms are requested within the same assessment
