What to Expect

You are about to complete a secure online intake and payment form for a Disability Tax Credit (DTC) assessment with Insight Disability Solutions.

This process collects the information required for Nurse Practitioner review and includes a non-refundable assessment fee.

Once submitted and payment is processed, your information will be reviewed. A member of our team will contact you within 3–4 business days (excluding weekends and holidays) to confirm next steps and arrange the required secure virtual consultation.

Please note: completion of this form and payment does not guarantee approval of the Disability Tax Credit by the Canada Revenue Agency (CRA).

If you have questions before proceeding, please contact our office prior to submitting the form.

For the best experience, we recommend completing this process on a laptop or desktop computer. Mobile devices may be used if needed.

Before You Begin

Please ensure you have 10–15 uninterrupted minutes to complete this process.

This assessment involves two required steps:

  1. Initial Intake & Payment

  2. Functional Intake Form, accessed immediately after payment

The link to the second form is provided on the confirmation (Thank You) page after payment and should be completed in the same session whenever possible.

We are unable to proceed or schedule a virtual appointment until both forms have been submitted.

If the second form is not completed at that time, access may be delayed and your assessment cannot move forward.

If You Need to Finish Later

If you are unable to complete the second form immediately, please ensure you save or bookmark the link provided on the confirmation page.

If the link is closed or lost, access to the second form will need to be re-issued via email, which may delay your assessment.

Your virtual appointment will be scheduled only after both forms are completed. Once submitted, we receive instant notification and will work on your next steps in a timely manner.

How the Disability Tax Credit Assessment Works with Insight Disability Solutions

Here’s what to expect once you begin:

  1. You click the Start Your Assessment button on our website (or receive a secure link by email)

  2. You complete the Initial Intake & Payment Form

  3. You are securely redirected to Stripe checkout

  4. Payment is completed successfully

  5. Stripe confirms payment

  6. Your intake submission is finalized in our system

  7. A confirmation page appears with next steps and a link to the second required form

  8. You complete the Functional Intake Form

  9. A Nurse Practitioner is notified and reviews your information within 3–4 business days

  10. We contact you to book your virtual appointment once both forms are received

This process ensures your assessment is efficient, secure, and appropriately prepared prior to your visit.
