At Insight Disability Solutions, we provide licensed healthcare provider-led virtual assessments and professional documentation to support applications for adaptive equipment and vehicle accessibility modifications.

Standard Rate

$150 per request
Includes:

  • Virtual or phone functional assessment

  • Professional verification and recommendation for adaptive equipment or vehicle modifications

  • Completion of the required application form or documentation

  • Guidance on submission to the program or funding source

Multiple Requests / Add-On Options

  • Second request during the same assessment session: $75

  • Additional requests beyond two: an additional $50 per document

  • Bundled requests must be submitted within the same session

For clients who are no longer under active care with their oncology team or cannot see their primary medical provider, we provide professional, virtual support to help you access the devices and supports you need to maintain independence and quality of life.

What We Do

We assist with functional documentation for a variety of post-surgical and medical assistive devices, including but not limited to:

  • Medical wigs

  • External breast prostheses / post-mastectomy bras

  • Post-surgical support garments (compression garments, abdominal binders, surgical bras)

  • Temporary mobility and self-care aids (walkers, shower chairs, dressing aids)

We focus exclusively on functional impact documentation, ensuring your forms clearly communicate your needs for insurance, benefit, or community support purposes.

Note: We do not provide medical treatment, prescribe devices, or make clinical recommendations. Our service is strictly documentation and functional assessment.

Why forms are needed:

  1. Insurance Reimbursement

    • Private health insurance or extended health benefits often require a healthcare provider’s note or prescription confirming the wig is medically necessary due to hair loss from medical treatment (usually chemotherapy or alopecia related to illness).

    • The provider documents the functional or medical impact of hair loss, which insurers use to approve reimbursement.

  2. Tax or Disability Credits (less common)

    • In some cases, a medically necessary wig can qualify as a medical expense for tax purposes. A supporting form from a healthcare provider makes this claim valid.

  3. Community or non-profit programs

    • Some provincial or charity programs (like wigs for cancer patients) may request a doctor or nurse practitioner form verifying medical need.

  5. Submission Support: We provide instructions on submitting to insurance or benefit programs and remain available for questions, renewals, or clarifications.

 

Pricing

  • Single Device Form / Letter: $75

  • Additional Devices (same assessment, same time period): +$25 each

Pricing includes virtual functional assessment, professional documentation, and guidance through submission.
